IMPORTANT HEALTH COVERAGE DOCUMENT – FORM 1095-C
If you were eligible for minimum essential health coverage through the Independent Bank Corporation Health and Welfare Plan in 2025 (including former employees and other covered individuals), you are entitled to receive Form 1095-C as proof of the coverage we offered you. Due to updated regulations that reduce paperwork, we are no longer required to mail paper copies.
You do not need Form 1095-C to file your taxes. If you would like a free copy for your records, a digital copy will be provided by email. To request a copy, please email humanresources@ibcp.com or call Human Resources at 616-306-8059.

If you would prefer a paper copy by mail instead, you may request one by emailing humanresources@ibcp.com or by writing to: HR, 4200 East Beltline Avenue NE, Grand Rapids, MI. A paper copy will be mailed to the address on file within 30 days of your request.
